I am voluntarily applying to attend the Partners Retreat through KFM to Tel Aviv, Israel for religious purposes.*

I acknowledge that transportation from my country of residence to Tel Aviv (only for non-flight bookings), passport or visa applications, passport charges, deposit and cancellation insurance, luggage insurance and related matters, otherwise not covered as part of the trip package of my booking are my sole responsibility.*

I agree that should I wish to make a cancellation or withdrawal during the retreat, I will produce a 30 days written notice to KFMI, and if I cancel the trip less than the notice period before the date of departure I will be charged a minimum of 30% cancellation fee. I also note all flight packages will have a cancellation fee irrespective of period of notice given*
